Patient Assistance Transport Scheme (PATS) Application Form and Handbook


To receive a PATS subsidy, the Country Health SA Local Health Network requires patients to seek authorisation from their Local Doctor in the first instance, visit their Specialist, and then after the medical appointments, complete and send the relevant sections of the application form below, to the nearest PATS office. 
The application form is available in three sections:

Section 1 for Local Doctors (170Kbs-PDF)  

Section 2 for Specialists (200Kbs-PDF)

Section 3 for Patients (200Kbs-PDF) 

Section 1 is required for the first PATS claim and may be optional for future claims, e.g. where patient payment details have changed or air travel was medically necessary.  Section 2 and 3 are required to be sent to a PATS office for every PATS claim. 

Applications forms must be lodged within three (3) months of the first specialist appointment.

Please note that previous versions of the Application Forms including the brown PATS ‘Application for Assistance’ form and the interim grey PATS ‘Country Health SA Hospital Inc Patient Assistance Transport Scheme Application Form’ are not accepted.
