Section 3 for Patients (200Kbs-PDF)
Section 1 is required for the first PATS claim and may be optional for future claims, e.g. where patient payment details have changed or air travel was medically necessary. Section 2 and 3 are required to be sent to a PATS office for every PATS claim.
Applications forms must be lodged within three (3) months of the first specialist appointment.
Please note, from 1 November 2011, the brown PATS “Application for Assistance” form and the interim grey PATS “Country Health SA Hospital Inc Patient Assistance Transport Scheme Application form” will no longer be accepted.
